FINANCIAL POLICY


At Endodontic Health of Palo Alto, we make every effort to provide you with the finest care and the most convenient financial options.  


FOR THOSE PATIENTS WITHOUT DENTAL INSURANCE:

Before your appointment, one of our assistants will contact you to discuss your possible out of pocket expenses. Payment is expected at the time services are rendered. We accept cash, check, Visa and MasterCard.  A fee will be charged for returned checks.  Most cases referred to our specialty practice are complex and may require multi-appointments in order to complete the treatment.   


FOR THOSE PATIENTS WITH DENTAL INSURANCE:

If you have dental insurance, please bring your insurance information with you to the consultation.  Your dental insurance is a contract between you, your employer, and the insurance company.  We are not a party to that contract.  The responsibility of payment ultimately lies with the patient, not the insurance company.  As a courtesy, we will work hand-in-hand with you to maximize your insurance reimbursement for covered procedures, and we will file your claim on your behalf.  The collection of benefits from insurance companies is the patient's responsibility.  Most dental insurance benefits are subject to limitations, exclusions, deductibles, co-payments and maximum benefit coverage.  In order to obtain your dental benefits, your insurance carrier often requires us to provide information before initiating treatment (preauthorization) for more complicated dental procedures. 


If payment for treatment is approved, you will be required to pay the “Estimated Patient Portion” and any deductible at the time of service.  If you elect not to have your treatment preauthorized through your dental insurance  (some treatment does not require preauthorization) you will be required to pay for your dental treatment on the date of service.  If in the end, you have made an overpayment because of your insurance benefit payment, you will be reimbursed.  If your insurance company denies payment for any procedure because of a change in your benefits or eligibility, you are responsible for the full cost of the treatment.  


We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.  We accept cash, check, Visa and MasterCard.  A fee will be charged for returned checks. 
 

Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
